Payment will be accepted as the athlete signs in at the facility. Checks can be made out to Karl Sorby.

Where: Bottineau High School

This camp will be ran by DCB's basketball team and coaching staff. 

When: May 28th-30th, 2024 

8:00 AM - 11:00 AM Grades 7-12 

11:00 AM - 12:30 PM Grades 1-3

1:30 PM - 3:30 PM Grades 4-6

Dakota College at Bottineau Sport Camp Participation Waiver - This form must be filled out completely prior to participation. Dakota College at Bottineau does not provide Accident/Medical Insurance for its campers. I understand the Dakota College at Bottineau Athletic Camp director and instructors will not be held responsible for injuries or loss of property while the above-named student is attending Dakota College at Bottineau Sport Camp. By checking the box, I hereby release the Dakota College at Bottineau, its officers, agents, and employees from any and all liability, including claims and suits in law or equity, for any injury, fatal or otherwise, or the loss of personal property, and will indemnify and hold harmless the Dakota College at Bottineau, its officers, agents and employees from any such claims. I realize the risks involved to the student, including the risks inherent to the sport camp. I will pay, or cover through my insurance, any medical or hospital expenses, doctor bills, or other expenses that could be incurred as a result or treatment given the student named on this online registration for illness or injury while attending this Dakota College at Bottineau Sport Camp. I hereby authorize the staff at the Camp to act for me according to their best judgment in any emergency requiring medical attention.
